I don't think you can go wrong with reading Christian Astrology, one nice thing about Lilly's work is that he summarises the tradition which went before him, and then offers several example charts to help follow along with the process once you understand the theory.

After that I personally like Bonatti's treatise on horary, as I find it a little clear in terms of technique, but we lack the examples we get from Lilly.

If you have the inclination to buy two, these are the ones I would personally favour.

As for how to make questions, my personal approach would be to imagine that you have some limited quota of horaries you're permitted - you would only cast the chart when you really really have to. In other words, you wouldn't 'waste' your quota on frivolous questions, you'd try to keep it for when you really need it, when you really have some anxiety or problem you need addressed.

Of course in reality there is no quota, but it can be helpful to think in these terms to ensure that you really keep horary for when it's needed, because once you do that, the symbolism just seems clearer and more relevant to the issue.

So cast a chart infrequently. It would be useful to learn the basic theory first, how to assign significators etc. and then just keep an antenna out there for when you realise that you have need of some extra clarity or help, and you may realise you could think the problem in horary terms - if you were to ask the question what would be the implications of a yes or no (for example), what would you do based on those implications. When you feel that you actually understand the question fully and you understand what exactly it is you need clarity on or you need to know, then you realise that this is the moment to cast the chart, you understand the question and you're ready to answer it, and you have some understanding of the theory of how to go about doing it. So you note your time and location and cast the chart accordingly.
